A new short-term care program for Aboriginal and Torres Strait Islander people aged over 50 will be available for Geraldton residents requiring nursing care.

Geraldton Regional Aboriginal Medical Service (GRAMS) have introduced a Transition Care Program designed to provide elderly First Nations people with short-term care after receiving hospital treatment.

The program will provide up to 12 weeks of care for clients over the age of 50 who require assistance with daily tasks at home after hospital discharge.

The program is designed to provide safe and high-quality cultural specific care for Aboriginal or Torres Strait Islander people.

Care will include nursing care, home cleaning, personal hygiene, meal preparation, facilitating social activities and health-maintenance therapy.

TCP referrals may only come through the Geraldton Regional Hospital from an Aged Care Assessment Team.
